Intake

We want this process to be smooth and we are here to help you in any way possible.  If you have questions, please feel free to call the office with any questions you may have when completing your new patient documents. 

  • ​Please bring your photo ID, completed paperwork, and insurance card(s) with you to your appointment.  If you have questions regarding your insurance benefits, please contact your insurance carrier at the number listed on the back of the card.

  • If you are completing your intake using Telehealth, an image of your photo ID and insurance cards can be uploaded using the Patient Portal.  If you need to complete your documents by paper, arrangements can be made with the clinician to have the documents emailed or mailed to you. You can return your completed paperwork by uploading it to the secure client portal, mail, fax, or scan and email to janels@thenextchapterICT.org

  • All co-pays, co-insurance, and deductibles are due at the time of service. 

  • We will confirm your appointment by text to the phone number and/or email address you provide.  If you are unable to attend your scheduled appointment, please kindly give us a 24-hour notice.

  • You should get an email from our client portal with all the intake documents to complete at your convenience.

  • All intake documents must be completed and provided to clinician prior to initial appointment.
